1. On the form, please enter
   a. The specific card number,
   b. In Honor or In Memory of Name(s),
   c. The Name(s) and Address of the Recipient,
   d. Any Special Requested Language Changes.
2. Enter your name, mailing address, email address and contact information.
3. Check the amount you intend to donate to the BallenIsles Charities Foundation.
4. REMEMBER: You can select whether you intend to pay by check or by your CLUB ACCOUNT. If you select club account, you will be asked to enter your member number.
5. After you submit the form, we will email you a confirmation of your order.
6. NOTE: Please make donation checks payable to the BallenIsles Charities Foundation. You can mail your check to us at 100 BallenIsles Circle, Palm Beach Gardens, FL 33418 or drop off at the Concierge Desk in the Clubhouse.
